Simplify bare defaults API by removing complex option_bool functions
Remove all the confusing option_bool variants (missing_false_bare_true,
missing_none_bare_true, etc.) and provide a clean, simple API instead:

1. Built-in helpers: bare_defaults::bool::{bare_true, bare_false}
2. Clean macro: bare_default!(fn_name, bool, default_value)
3. Use regular bool fields with #[serde(default)] for missing field handling

The new API is much easier to understand and use:
    bare_default!(writable_deser, bool, true);

    #[serde(default, deserialize_with = "writable_deser")]
    writable: bool, // missing → false, bare → true

Updated all tests, maintained 100% coverage, much better UX.

+//! You can specify custom defaults for bare node names (nodes without arguments):
+//!
+//! #### Built-in Boolean Helpers
+//!
+//! For simple cases, use the built-in functions:
 //!
 //! ```rust
 //! use serde::Deserialize;
 //!
 //! #[derive(Deserialize)]
 //! struct Config {
+//! // Required fields (must be present)
 //!     #[serde(deserialize_with = "serde_kdl2::bare_defaults::bool::bare_true")]
 //!     enabled: bool,
-//!
+//!
 //!     #[serde(deserialize_with = "serde_kdl2::bare_defaults::bool::bare_false")]
 //!     debug: bool,
 //! }
 //! ```
 //!
+//! #### Custom Defaults with the `bare_default!` Macro
+//!
+//! For more control, create custom deserializers:
+//!
+//! ```rust
+//! use serde::Deserialize;
+//! use serde_kdl2::bare_default;
+//!
+//! // Generate a custom deserializer where bare nodes → true
+//! bare_default!(writable_deser, bool, true);
+//!
+//! // Custom default function for missing fields → true
+//! fn enabled_default() -> bool { true }
+//!
+//! #[derive(Deserialize)]
+//! struct Mount {
+//!     source: String,
+//!
+//!     // missing → false (default), bare → true
+//!     #[serde(default, deserialize_with = "writable_deser")]
+//!     writable: bool,
+//!
+//!     // missing → true (custom), bare → true
+//!     #[serde(default = "enabled_default", deserialize_with = "writable_deser")]
+//!     enabled: bool,
+//! }
+//! ```
+//!
 //! ```kdl
-//! enabled          // → true
-//! debug            // → false
-//! enabled #false   // → false (explicit override)
-//! debug #true      // → true (explicit override)
+//! source "/path"           // writable → false, enabled → true (missing defaults)
+//!
+//! source "/path"
+//! writable // writable → true (bare default)
+//! enabled // enabled → true (bare default)
+//!
+//! source "/path"
+//! writable #false          // writable → false (explicit)
+//! enabled #false           // enabled → false (explicit)
 //! ```
 //!
-//! For other types, you can create custom deserializer functions following
-//! the same visitor pattern (see examples for details).
+//! The macro works for other types too - just implement the appropriate visitor methods.
